Clinicards can be used in a wide variety of clinical areas and offers a scalable digital health enviroment
The origins of Synappz lie in Life Sciences and business. It is built on a solid foundation of over 20 years of experience in R&D, human and veterinary healthcare, and the business side of the pharmaceutical industry. Starting with the dream to bring digital health in the hands of the patient, Synappz developed the first CE-certified medical app in 2012. Since 2016 they have been working on Clinicards, a scalable digital health platform to monitor, inform and coach patients with any kind of condition.
Clinicards focuses on improving care by monitoring, informing and coaching the patient and reducing healthcare costs. To collect patients’ data – such as blood pressure, pain scores, or even pictures of a wound – the app uses HealthCards. These HealthCards are managed and entered by the patients. In turn, healthcare providers can view their data on their Clinicards Portal.
To inform patients, Clinicards ContentCards provide patients with relevant and reliable information (articles, videos and animations) about their procedure and medical condition, and their CarePath shows the patient a clear overview of their patient journey.
The Clinicards app makes health a shared responsibility of the healthcare organisation and the patient. The collection of different HealthCards make it easier for a doctor to collect (medical) information about their patient, saving them a lot of time.
The ContentCards and CarePath allows the doctor to inform the patient with the relevant information at the right time, preventing a patient from becoming overloaded with information. This results in better informed patients.
The Clinicards platform can be used in a wide variety of clinical areas. The scalable solution not only reduces the health costs, but also makes the care more efficient, personal and more human for the patients.
Healthcare needs a strong digital component in order to be able to deal with future developments. Whether it is in remote monitoring to keep track of health, providing the right educational content to prepare for an intervention, or coaching towards a certain health outcome. Clinicards is a versatile platform that allows healthcare organisations to deliver digital health in these areas.
With Clinicards’ unique modules Cards, Content, CarePath, CareGiver, Communication, Community and Connection it is possible to set-up a full-fledged, scalable digital health environment with a mobile app for patients and a portal for healthcare professionals.
