About Synappz
Synappz's Clinicards focuses on improving care by monitoring, informing and coaching the patient and reducing healthcare costs. To collect patients’ data – such as blood pressure, pain scores, or even pictures of a wound – the app uses HealthCards. These HealthCards are managed and entered by the patients. In turn, healthcare providers can view their data on their Clinicards Portal.
To inform patients, Clinicards ContentCards provide patients with relevant and reliable information (articles, videos and animations) about their procedure and medical condition, and their CarePath shows the patient a clear overview of their patient journey.
ContentCards and CarePath within our Clinicards platform enables the healthcare provider to specify which information should be shown on the patient's app and when.
What problems does Synappz solve?
The Clinicards app makes health a shared responsibility of the healthcare organization and the patient. The collection of different HealthCards make it easier for a doctor to collect (medical) information about their patient, saving them a lot of time.
The ContentCards and CarePath allows the doctor to inform the patient with the relevant information at the right time, preventing a patient from becoming overloaded with information. This results in better informed patients.
